Membership schemes under "Eye Care For Life Time":

TYPE 1

CORPORATE MEMBERSHIP

In this any group of people, companies or clubs can take a corporate membership by paying Rs.10,000/-. The members of the corporate then can avail of the advantage of eye care for lifetime by paying only Rs.1250/- and this will allow the member and the spouse to avail of the concessions. Even if the member leaves the corporate he still continues to have the benefit. The corporate also does not have to pay again and any new employee's member will automatically get the benefit of becoming member of the above scheme.

TYPE 2

EMPLOYEE BENEFIT SCHEME

The corporate can decide to offer comprehensive eye care cover for its employees at a nominal cost per month. This scheme is available for corporate where a minimum of 250 people need to be covered. The typical costs range from Rs. 20 per employee per month to Rs. 13 per employee per month based on the number of employees covered.

Type 3

Family and Individuals are covered at Rs 2000 per couple

ANNUAL CHECK UP AT CORPORATE PREMISES

An on site check up facility for an unlimited number of employees is available at a nominal cost of Rs 50 per employee.

What we cover in the Membership scheme :

This scheme covers consultations and professional services of the Ophthalmologist for all eye surgeries, treatment, procedures during the entire lifetime of the members.

If the members exceed 100 from a single corporate, the consultations can be done at the company premises once in a year in addition to the services we provide when the member visits our hospital

Workshop on eye care once a year

All membership fees collected under these schemes will go towards providing free services to the needy.
